How to Become a Florida Medicaid Provider
Follow these four steps to apply for your Florida Medicaid provider license.
Step 1: Start the application
Start your application online through the Florida Medicaid Provider Enrollment portal. You’ll need to provide the following information:
- Personal and business information
- Tax ID
- National Provider Identifier (NPI)
- Copies of professional licenses and certifications
- Provider type and specialty codes
- Letter from the depository bank, or a voided check, verifying account information (if you will receive direct Medicaid payments)
- Supporting documentation for any adverse history (if applicable)
Step 2: Schedule a background screening
All owners, managing employees and board members must undergo Level II background screening through an AHCA-approved vendor. The screening involves fingerprinting and a criminal background check.
Step 3: Submit additional documents
Depending on your provider type, you may need to include supplemental documentation, such as proof of liability insurance or agreements with subcontractors. Once everything is attached, submit your completed application through the enrollment portal.
Step 4: Await approval
Once submitted, the AHCA will review your application. You’ll receive your provider license once all requirements are met. The approval process usually takes 30–60 days, depending on the status of your background screening results.
